Disability Related Health Supports

We believe a seamless connection between medical, behavioural, communication and therapeutic supports is crucial for participants to receive truly holistic care. From medication management, health assessments, and plan formulation to training support workers and caregivers, our nurse Romy provides an integrated approach to participant care. Additionally, she collaborates with Positive Behaviour Support Practitioners, Speech Therapists, and Occupational Therapists, bridging the gap between clinical and therapeutic supports.

Romy's Nursing Supports

Meet Romy, our clinical nurse. Romy brings over 24 years of nursing experience at the Royal Adelaide Hospital (RAH) after graduating in 1996.

Seeking to make a real change, Romy prides herself in her holistic approach. She ensures participants and their families receive the best possible support for their healthcare needs.

 

After a health assessment, Romy will create a care plan to be implemented by trained support workers and others if required. Romy ensures you receive ongoing support and will update care plans and trainings as needed.

At Blossom we can provide Disability-Health Supports (High Intensity Supports) and management for:

  • Seizure Management
    Monitoring and managing seizures, including medication administration and recovery.

     

  • Dysphagia Supports
    Help with eating, drinking, and swallowing difficulties.

     

  • Continence Care
    Help with toileting, bowel care, and continence management.

     

  • Nutrition and Meal Preparation
    Guidance on eating well and preparing meals to maintain health and well-being.

     

  • Respiratory Supports
    Assistance with respiratory care, planning, and oxygen management.

     

  • Training for Carers and Families
    Tailored training to support clinical requirements and ensure effective care.

     

  • Diabetes Management
    Support with tasks like blood sugar testing for participants unable to manage this independently.

The Process

  1. Individualised Clinical Health Assessment
    A personalised clinical health assessment is conducted to identify the participant’s specific care needs and the level of support required.
     

  2. Development of a Care Plan
    Based on the assessment, a detailed care plan is written and shared with participants or families for review. The care plan is designed to be flexible, allowing updates and adjustments based on the participant’s clinical changes or at the family’s or carer’s discretion.
     

  3. Tailoring Services
    The care plan helps determine the most suitable team to deliver the required services, ensuring the plan is tailored to the participant’s individual needs.
     

  4. Training for Support Workers
    For Blossom high intensity participants, support workers are trained and educated on all aspects of the participant’s clinical requirements before care is implemented.
     

  5. Implementation of Care
    Care is delivered in accordance with the approved care plan, ensuring the participant’s needs are met safely and effectively.
     

  6. Ongoing Review, Feedback and updates
    Families are contacted regularly to review the progress of supports, ensuring they align with the participant’s needs and goals.
    Adjustments to the care plan are made as required to reflect any changes or feedback from families or carers.
