Over the past year, while working alongside our nurse with our high intensity participants, we’ve identified a significant gap between the care and supports people receive and their medical needs. Coordinating clinical and medical appointments, updating care plans, managing medication changes, sharing vital information with multidisciplinary teams, and ensuring all are properly trained can often be a challenging and overwhelming process. A seamless connection between medical, behavioural, communication and therapeutic supports is crucial for participants to receive truly holistic care Romy with her son, Lincoln (who has special needs) prior to a day surgery procedure. That's where Romy, our Clinical Nurse, steps in Romy has been working closely with participants and families, connecting all pieces of their care information, ensuring a seamless link between medical, behavioural, communication and support needs. Romy brings over 24 years of healthcare experience, after graduating in 1996 with a Bachelor of Nursing, Romy has had a distinguished career at the Royal Adelaide Hospital (RAH). Romy’s holistic approach ensures participants and families receive the best possible support for their healthcare needs. Romy's experience inclues: Perioperative and Critical Care Specialising in operating room services, airway management, and recovery. Educator and Leadership As the Former Head Educator for Operating Room Services, Clinical Facilitator, and Basic Life Support Instructor. Specialist Areas such as Medical oncology, plastics/craniofacial care, and staff development. Seeking to make a real change, Romy prides herself in her holistic approach. She ensures participants and their families receive the best possible support for their healthcare needs. From medication management, health assessments, and plan formulation to training support workers and caregivers, Romy provides an integrated approach to participant care. Additionally, she collaborates with Positive Behaviour Support Practitioners, Speech Therapists, and Occupational Therapists, bridging the gap between medical and other supports. Nursing Supports at Blossom At Blossom we can provide Disability-Health Supports and management for Seizure Management Monitoring and managing seizures, including medication administration and recovery. Dysphagia Supports Help with eating, drinking, and swallowing difficulties. Continence Care Help with toileting, bowel care, and continence management. Nutrition and Meal Preparation Guidance on eating well and preparing meals to maintain health and well-being. Respiratory Supports Assistance with respiratory care, planning, and oxygen management. Training for Carers and Families Tailored training to support clinical requirements and ensure effective care. Diabetes Management Support with tasks like blood sugar testing for participants unable to manage this independently. Our Clinical Nurse, Romy may also assist you with Assessments, creating, implementation and training in your health care plans Accessing and navigating the healthcare system Medication safety and awareness Skin integrity Mobility and falls risk management Sleep Apnoea Coordinating multidisciplinary teams Health monitoring and check ins Pre- and post-operative care Accessing and navigating the healthcare system Communicating changes to your health care needs High Intensity Supports High-intensity supports cater to participants with complex medical needs. Unlike standard supports, these require specialised training and oversight. We focus our energy on ensuring this quality remains and staff/participants are trained and have adequate support plans. Support workers for high-intensity participants undergo additional training to ensure safety and quality care when working with participants whom have - Dysphasia care plans - Epilepsy Management Plans - PEG regimes - Complex bowel care - Oxygen Management The Process Individualised Clinical Health Assessment A personalised clinical health assessment is conducted to identify the participant’s specific care needs and the level of support required. Development of a Care Plan Based on the assessment, a detailed care plan is written and shared with participants or families for review. The care plan is designed to be flexible, allowing updates and adjustments based on the participant’s clinical changes or at the family’s or carer’s discretion. Tailoring Services The care plan helps determine the most suitable team to deliver the required services, ensuring the plan is tailored to the participant’s individual needs. Training for Support Workers For Blossom participants, support workers are trained and educated on all aspects of the participant’s clinical requirements before care is implemented. Implementation of Care Care is delivered in accordance with the approved care plan, ensuring the participant’s needs are met safely and effectively. Ongoing Review, Feedback and updates Families are contacted regularly to review the progress of supports, ensuring they align with the participant’s needs and goals. Adjustments to the care plan are made as required to reflect any changes or feedback from families or carers. Funding Details Clinical nursing supports are funded under Capacity Building – Improved Daily Living , within the NDIS category of Disability-Related Health Supports. The specific support items include: Delivery of Health Supports by a Clinical Nurse 15_412_0114_1_3 Delivery of Health Supports by a Registered Nurse 15_406_0114_1_3 Ongoing Costs for clinical nursing may include: Healthcare plan reviews and updates. Additional Training Costs of advanced training for high-intensity support workers. Clinical interventions as required. - High Intensity Supports
High-intensity supports cater to participants with complex medical needs. Unlike standard supports, these require specialised training and oversight. We focus our energy on ensuring this quality remains and staff/participants are trained and have adequate support plans. Support workers for high-intensity participants undergo additional training to ensure safety and quality care when working with participants whom have: - Dysphasia care plans - Epilepsy Management Plans - PEG regimes - Complex bowel care - Oxygen Management These participants require support workers with more skills and competencies along with oversight by a registered nurse. Roll out of High Intensity Support The NDIS funds the above disability-related health supports that are linked to a participant's significant and lasting functional impairment. These tailored supports, delivered by a Level 2, Disability Support Worker, are available in various settings, including the community and the participant's home. Since our registration, we will be commencing the roll out of high intensity supports to participants requiring these supports. Some participants may require a combination of standard and high intensity supports, depending on their needs. The NDIS will include include enough funding for a support worker for the times when you receive the support. They will account for the cost for the support worker to be trained by the registered nurse and provide the delegated tasks. **For current Blossom participants, we will be transitioning to high intensity support prices at the beginning of 2025!** Accessing High Intensity Supports You will be supported by our Nurse, who will work in collaboration with you, your caregivers and your team, during the below process. We may also hire aditional people or train aditional staff to support you specifically. Training for Support Workers Support workers are trained and educated on all aspects of the participant’s clinical requirements before care is implemented. Implementation of Supports Care is delivered in accordance with the approved care plan, ensuring the participant’s needs are met safely and effectively. Ongoing Review, Feedback and updates Families are contacted regularly to review the progress of supports, ensuring they align with the participant’s needs and goals. Support workers are regularly supported by the nurse and line manager. Adjustments to the care plan are made as required to reflect any changes or feedbacks. Funding Details Clinical nursing supports are funded under, Core - Daily Living and Social and Community Participation, and Capacity Building – Improved Daily Living , within the NDIS category of Disability-Related Health Supports.
